6 killed, 30 injured September 18, 2013 Train Hits Canada Bus Crashing Through Crossing Barrier Six are killed in Ottawa Canada when a double-decker transit bus blasted through the crossing gates to hit a passing passenger train. The driver was killed as the nose of the bus was ripped off and derailed the train. 30 were injured, 11 critically, but there were no injuries on the train. Bus passengers screamed at the driver who did not slow down and appeared to be intent on driving into the train, they wondered if he had passed out or did not see the train or the closed gates. The bus operator said that the cause of the crash was not clear. Witnesses say that driver David Woodard ignored warnings, but investigators will not "jump to conclusions". The bus driver had a good driving record, and had a prescription for Metformin, a drug used to treat people with Type 2 Diabetes, a common disease in bus drivers.

In 2014 Transportation Safety Board officials revealed that speed and visual distraction may have contributed to the deadly collision. Preliminary findings show the bus was travelling about 8 kilometres over the speed limit in the lead up to the crash, and safety board investigators believe Woodard may also have been glancing at video monitors showing the second-floor of the bus. Investigators have apparently not thought of the possibility of suicide. Clemy Srour and others are among 5 lawsuits against the City of Ottawa as well as the estate of bus driver David Woodard.

Bus jihad. Driver error? No other way to explain driving through crossing gates.

This comment on bloombergh hits it on the head: This seems very suspicious to me. It sounds like the bus driver wanted to commit suicide. Why else would he keep going in spite of the lowered gate, flashing lights, and screaming passengers. Who on earth would be oblivious to all that and just keep driving? Did he try to stop at the last minute, but couldn't because, ya know, it's a bus, so you can't stop on a dime. The article made it sound like he made absolutely no attempt to stop. It sounds like he didn't want to stop

The Ottawa bus-train crash was a collision that occurred between a OC Transpo double-decker bus and a Via Rail train in the Ottawasuburb of Barrhaven on September 18, 2013, that killed six people.[1][3]

The collision happened between a route 76 OC Transpo Alexander Dennis Enviro500 double-decker bus (identification number 8017) and Via Train 51 (identification number 915), headed for Toronto, at 8:48AM.[1][2][4][5] It occurred at a rail crossing near Fallowfield Road and Woodroffe Avenue on the Transitway, the city's dedicated road for buses.[1][4] The level crossing's crossing gates, lights, and bells engaged 47 seconds before the crash, and its gates were fully horizontal across the road 25 seconds before the crash.[4] The train was travelling at 75 kilometres per hour, in an area where the speed limit is 161 kilometres per hour, at the time of the collision and had engaged its emergency brakes two seconds prior.[4] The collision sheared off the front end of the bus and derailed the train.[1][5] Five people were pronounced dead at the scene of the accident, including the bus driver, and one later died in hospital.[1] A further thirty-four people were injured on the bus.[2] No serious injuries occurred on the train.[1] Canadian Prime Minister Stephen Harper as well as Ontario Premier Kathleen Wynne were among those who offered condolences to the victims of the crash and their families.[5] The bus and the train involved in the crash were both removed from the scene on September 19, 2013.[6] Via Rail train service resumed through the area in the morning of September 20, 2013.[3] A community memorial was held for all of the victims of the crash at Cedarview Alliance Church on September 22, 2013, where funerals for two of the victims were also held.[7]

Investigation[edit]

The crash is currently being investigated by the Transportation Safety Board of Canada (TSB).[1] On September 28, 2013, TSB investigators and Ottawa Police investigators returned to the site of the incident in order to reenact the crash and collect more information on what might have led to collision.[8] On October 16, 2013, based on early observations made by the TSB, Ottawa’s transit commission, in the body's first meeting since the crash, outlined four new safety improvements for the level crossing that was the site of the crash.[9][10] These measures were clearing trees and brush from the area of the crash site to improve visibility, reducing the speed along the Transitway approaching the level crossing from 60 to 50 kilometres per hour (37 to 31 mph), installing bigger signs, and installing a new early warning light to caution buses when a train is coming.[9][10] The city stated that these measures would only be extended to the crossing where the crash occurred and that these observations made by the TSB should not be confused with causes of the crash.[9][10]

All six victims of Wednesday's crash have been identified by Ottawa Police.
Police identified the victims of the crash as Michael Bleakney, 57, Connor Boyd, 21, Karen Krzyzewski, 53, Kyle Nash, 21, Rob More, 35, and Dave Woodard, 45.
Woodard was driving the bus that passengers and witnesses said went through the safety barriers and flashing lights to strike the VIA Rail train at a busy Transitway crossing.
But the 11 Transportation Safety Board investigators now assigned to the crash won't jump to any conclusions, spokeswoman Julie Leroux said.
"Right now, we investigate all the possibilities," she said.
"We haven't ruled out anything yet. We are going to go through all the information we have and see where the investigation will lead us."
