It was thought to be a mechanical failure, but investigators also considered that someone could have deliberately set it in motion, as it requires a key to get into the driver booth, another key to start the train. A driver is required to release the brake, and a dead-man switch stops the train if there is no operator. There are switch safeguards to keep the train from entering the main track.

The train did not start itself. Somebody had to start it.

You don't need evidence of who did it to figure out that somebody did it, and it doesn't take much guessing what sort of people might have had the motive and capability to do it if they had somebody do an inside job.

Multiple safety systems had to be deliberately defeated:
not one, but two keys are needed to operate the train. 
you would need a CTA universal key for the cab door and a second one to operate the master controller.
Without a driver, the friction brakes would be on while the train would be stationed in the yard. 
Even if the key turns power on in the train, a human operator needs to release the brakes. 
That is what CTA operators call a “dead man control,” and the operator would need to use both hands to deactivate the dead man switch.

A reconstruction video of yesterday's train crash in Chicago shows the moment of impact between a runaway train, with no driver aboard, and a stationed train filled with passengers. 
The clip has been uploaded by Malay Mail Online. The “ghost train” rode for half a mile (0.8 km) with nobody noticing before crashing, Chicago Tribune reported.

 Robert Kelly, president of the CTA rail workers union, gives more details on the Chicago Train Crash. He explains that it is hard to believe that this is a malfunction, as not one, but two keys are needed to operate the train.  He elaborates that you would need a CTA universal key for the cab door and a second one to operate the master controller.

 That cannot be done while moving, because it would turn off an automatic brake system and the train would stop.

- A Chicago commuter train that was parked in a service yard somehow moved onto a rail line and smashed into an oncoming train early Monday, injuring dozens of people.
How it happened is a mystery. "I don't know the last time, if ever, that this has happened" on the Chicago Transit Authority system, spokesman Brian Steele said.
There are "more questions than answers" about the incident in Forest Park, Illinois, he said.
At least 48 people were injured; 33 of them were transported to hospitals, Forest Park Mayor Anthony Calderone told CNN affiliate WLS. They are believed to have minor injuries.
