He could easily have ended an amputee or a quad as killing himself. It's possible there was a mechanical issue or that he fell asleep. Apparently, the union has been complaining about new work schedules that were putting tired men at the controls. At this point, we don't know if malfeasance, error, mechanical problems, sabotage or fatigue was to blame.
