March 9, 2010 Gas Pedal Confusion Rams Propane Tanks at K-Mart In Camarillo California, a 78 year old driver Patricia Oakman Ashbrook accidentally hit the gas pedal of her Toyota Avalon in reverse, damaging three cars. Then she accidentally shifted into drive at full throttle, and accidentally ramming a rack of highly inflammable filled propane tanks which did not explode. A shaken and shocked witness described hearing "squealing and the crunching, like she was doing 80 to 90 mph". Officers blamed the driver as the car checked out with no problems.

Revving an engine at high speed is usually an indication of an intent to attack a target, but this was just an accident.

September 7, 2014  0 injuries, building destroyed Explosion Destroys Kentucky Hotel  A Kentucky hotel was destroyed in an explosion Sunday, after a driver "accidentally" drove into a gas meter, authorities said. The explosion blew out the front wall and caused collapse of one wing of the 3 floor building at the Fairfield Inn & Suites in Lexington. No one was injured. Alternative theory - perhaps this was not an accident given the number of people who have accidentally driven down stairs, into doors and into propane tanks and bedrooms of sleeping children. The fire department got a call of a private alarm at the Fairfield Inn about 6:15 p.m. Sunday and arrived the staff evacuating guests because the gas was leaking. About 10 minutes later, there was an explosion. Several cars were damaged and were under rubble.  An older man thought he hit the brake but instead hit the gas and drove into an exposed gas meter,which officers quickly ruled an accident.  youtube video
