March 24, 2014. Driver Claims Sleeping Caused Train To Ram Into Escalator at Chicago Airport At the Chicago O'Hare airport station, a an eight-car train rammed beyond the stop with such force that it climbed the escalator adjacent to the passenger platform. The driver admitted she may have dozed off, and she was fired as it was her second safety violation.

December 1, 2013 2013 NYC Metro North Bronx Train Derailment , a Metro-North Railroad passenger train derailed near the Spuyten Duyvil station in the Bronx, killing 4 people and injuring 63 others.A freight train had derailed on the other side of the same station four months earlier.The NTSB found the train was going 82 miles per hour as it entered a 30-mph curve. Driver William Rockefeller 20-year veteran of the MTA and has clean disciplinary record was first heard to have said that the brakes did not work. Later the driver told first responders "he began to daze, thinking about nothing in particular and the train began to derail". He did not apply brakes until it was already derailing. Neighbor Tracy Pool, said Rockefeller is an avid motorcyclist who "likes like to live on the edge" but was always within speed with his trains.

September 30, 2013 Ghost Train Crash in Chicago Needed 2 Keys To Start In Chicago, an out-of-service train somehow was set into motion and traveled without a driver for a half mile before it slammed into a full train which had stopped at a station, injuring 48 people were taken to about 10 area hospitals. It was thought to be a mechanical failure, but investigators also considered that someone could have deliberately set it in motion, as it requires a key to get into the driver booth, another key to start the train. A driver is required to release the brake, and a dead-man switch stops the train if there is no operator. There are switch safeguards to keep the train from entering the main track.

September 18, 2013 Train Hits Canada Bus Crashing Through Crossing Barrier Six are killed in Ottawa Canada when a double-decker transit bus blasted through the crossing gates to hit a passing passenger train. The driver was killed as the nose of the bus was ripped off and derailed the train. 30 were injured, 11 critically, there were no injuries on the train. Bus passengers screamed at the driver who did not slow down and appeared to be intent on driving into the train, they wondered if he had passed out or did not see the train or the closed gates. The bus operator said that the cause of the crash was not clear. Victims of the crash were identified as Michael Bleakney, 57, Connor Boyd, 21, Karen Krzyzewski, 53, Kyle Nash, 21, Rob More, 35, and Dave Woodard, 45. Woodard was driving the bus who witnesses said ignored warnings, but investigators will not "jump to conclusions".
