August 27, 2015 Duluth's Sacred Heart Music Center damaged by runaway truck swerves to avoid traffic after brakes "accidentally" fail Hallowed ground to musicians around Minnesota, historic 1896 Sacred Heart Music Center in Duluth was struck by an out-of-control pick-up truck on Thursday, which caused an entire brick corner of the former church to crumble. the Duluth Tribune, the driver claimed his brakes went out, swerved into the building to avoid oncoming traffic.

May 16, 2014 Car With Parking Brake Off Rolls Into Garage, Gas Meter Explodes In Calabasas, California a neighbor’s car was parked on the hill apparently without its parking brake on rolled down the hill, crashed into the garage of a man named Brandon and knocked the gas meter loose, triggering the explosion. There were no injuries, but the house was a total loss.

Three grain cars had two of three cars brakes deliberately released, apparently an act of felony vandalism or sabotage. It caused the cars to run away, rolling past the end of a spur and derailing causing a crash with $30,000 in damage. In Canada, the Lac-Mégantic derailment destroyed the center of small town when oil cars ran away when their air brakes were released without explanation, and exploded when they derailed and burned. Terrorism was ruled out in that case.
